Completion of these details will allow us to carry out the smooth clearance delivery of your goods please complete both pages of this form and return to OSS with your documents as soon as possible. Delivery (may) be conducted via a courier to kerbside only. If so, you will be notified once your shipment has been scheduled to depart OSS warehouse. Pre-determined delivery times cannot be specified. You will be provided with a reference number so that you may contact the courier company regarding your delivery date and time. To avoid redelivery charges and storage fees, please ensure that either you or your nominated representative will be available to take delivery alternatively you may provide authority on this form to leave your items in a safe place specified by yourself on your property.

NOTIFICATION OF DELIVERY
Please nominate desired delivery date and any dates you will not be available to accept delivery. Please be aware clearance of your shipment will take approx. 10 days from vessel ETA date. Deliveries are conducted Monday to Friday between 7.00 am to 2.30 pm. Please advise if storage of your shipment is required.

AQIS ORDER TO CLEAN AT OSS WAREHOUSE
Australian Quarantine may require some items to be pressure cleaned due to soil or other biological contamination. As such we require your authorisation to clean these items, and also your acknowledgement that OSS or AQIS may not be held legally responsible for any damages incurred during or after the pressure cleaning.
Yes, I DO authorise OSS World Wide Movers to pressure clean any items ordered by Australian Quarantine to be cleaned because of soil contamination in accordance with AQIS instruction. I also acknowledge charges will apply for this cleaning and accept that OSS or AQIS may not be held legally responsible for any damages incurred during or after the pressure cleaning. Items most commonly ordered for cleaning are shoes ($71.50 up to 3 pieces). Estimated charges may vary from State to State and may range from $70.00 to $360 + GST dependent upon number of items and whether items are required to be cleaned by third party Quarantine accredited facility.
